Payment Policy & Insurance Information

PAYMENT

We expect payment of all fees, co-payments and deductibles at the time of your visit. We accept cash, personal checks (verified electronically), Visa, MasterCard, and Discover. If you do not have your insurance information with you at the time of your visit, you will be expected to pay in full for services rendered that day. We will prepare a claim form for you to file for reimbursement.

The Family Doctor participates with the following Insurance Companies:

* Private Health Care Systems

* United Healthcare

* Well Path

Please bring your insurance card(s) to every visit and show to our receptionist when you check in. Because insurance plans change frequently it is important to make sure that we have your correct insurance plan information. In order to use your insurance benefit, without extra out-of-pocket expense, it is your responsibility to notify us of any changes in your plan.

HMO members who are not on an Open Access Plan will need to have a referral faxed to us from your Primary Care Physician prior to treatment in our office. If a referral authorization is not obtained from your PCP prior to being seen, we will ask for payment at the time of service. Without an authorization, you may not be reimbursed for the services.

We do not participate with Medicaid or TriCare. Payment in full is expected from patients covered by these plans. You will not be reimbursed if seen in our practice, as we are non-participating providers.

We do not participate or accept assignment with Medicare. We offer a reduced rate for our Medicare patients and will file your insurance claim for reimbursement. Payment in full is expected at the time of service. At this time, we are not accepting new Medicare patients for ongoing medical care.

We are not participating providers with any commercial Medicare products. We do not participate with some commercial insurance plans. If we do not participate with your insurance plan, we will be happy to provide a claim form for you to file for reimbursement.

Worker's Compensation
& Company Accounts

Employers: Please contact our office to establish a company account with us. We participate with most major Worker's Compensation Insurance carriers.

We are happy to provide treatment for your workers comp injuries, pre-employment and post accident drug screens, TB testing, DOT physicals and pre-employment physicals.

After an account is established, you may call us to authorize treatment of your employees, or complete our Workers Comp Referral Form and fax it to (919) 942-0038 or send it directly with the employee to our office for treatment during all regular office hours.

*Employees must bring a LabCorp COC (Chain of Custody) form with them in order to perform post accident drug screens. Results from this testing is sent directly to the employer from LabCorp.
