Making an Appointment

To set up an appointment, please call our office at (919) 968-1985 x7 during our normal business hours of
M - W, 8 AM - 8 PM ; Th - F, 8 AM - 7 PM ; Sat, 8 AM - 6 PM ; and Sun, 1 PM - 5 PM .

For each office visit, we ask that you arrive at least 15 minutes early to update or complete paperwork. For example, you may need to update your contact information (address, home phone or workplace numbers) or your general health history form. If you are an adult man or woman having a complete physical exam, you will need to fill out a physical form. Children’s sports, camp, and kindergarten physicals also require completion of a questionnaire. Other types of office visits requiring completion of a questionnaire include: travel consultations, erectile dysfunction, DOT physicals, etc.

If you are having a yearly physical exam or plan to have your cholesterol (lipids) checked, please make a morning appointment so that you can come in fasting. Fasting means no food or drink (water is ok) for at least ten hours prior to your lab test.

If your appointment is for a school physical or travel consultation, please bring with you a record of your previous vaccinations.

We appreciate 24-hour notice if you are unable to keep a scheduled appointment. This will allow us to offer the time to another patient. We reserve the right to charge $50 (not reimbursable by insurance) for repeated no-shows or late cancellations of appointments.

If you are new to our practice, please arrive at least 20 minutes before your scheduled appointment in order to register, verify insurance information and to complete a brief general medical history form. We ask that you please bring ALL of the following to your initial appointment:
